A vaccination certificate issued in Japan must be one of the following:

• *Vaccination Certificate of COVID-19 issued by the Japanese government or a local government
• *Certificate of vaccination for COVID-19 issued by a local government
• *Record of vaccination for COVID-19 issued by a medical institution
• *Another certificate deemed equivalent to documents listed above

The following items must be indicated in Japanese or English: Name, date of birth, vaccine name/manufacturer, date of vaccination, number of doses administered.

• *If date of birth is not included in the certificate, it may still be considered valid if it contains your passport number or other information that can be used to verify that the vaccination certificate is yours.
• *If the vaccination certificate is in a language other than Japanese or English, it may be considered valid if it is accompanied by a Japanese or English translation and the content can be confirmed.

The certificate must clearly indicate that you have received two doses from among the following vaccines (vaccine name/manufacturer):

• COMIRNATY intramuscular injection/Pfizer
• Vaxzevria Intramuscular Injection/AstraZeneca
• Spikevax Intramuscular Injection /Moderna
• JCOVDEN intramuscular injection/Janssen
• COVAXIN/Bharat Biotech
• Nuvaxovid Intramuscular Injection/Novavax

• *For the JACOVDEN intramuscular injection/Janssen, one dose is considered equivalent to two doses. Hereafter the same.
• *Doses will also be considered valid even if both are not the same vaccine.
• *In applying measures based on New Border Measures (28), COMIRNATY manufactured by Fosun Pharma / BioNTech and Covishield / COVOVAX manufactured by the Serum Institute of India are treated as identical to COMIRNATY, Intramuscular Injection / Pfizer and Vaxzevria, Intramuscular Injection / AstraZeneca, Nuvaxovid, Intramuscular Injection / Novavax respectively.

The certificate must clearly indicate that you have received at a third dose, with it being one of the following vaccines (vaccine name/manufacturer):

• COMIRNATY intramuscular injection/Pfizer
• Spikevax Intramuscular Injection /Moderna
• Nuvaxovid Intramuscular Injection/Novavax
• Vaxzevria Intramuscular Injection/AstraZeneca
• JCOVDEN intramuscular injection/Janssen
• COVAXIN/Bharat Biotech (effective from July 31st, 2022)

• *Regarding the application of measures based on Japan’s Enhanced Border Measure (28), Comirnaty (manufactured by Fosun Pharma/BioNTech), as well as Covishield and COVOVAX (manufactured by the Serum Institute of India) will be treated as identical to Comirnaty/Pfizer, Vaxzevria/AstraZeneca and Nuvaxovid/Novavax, respectively.
• *Vaccines not listed in “(3) First and Second Dose” don’t count as the first or second dose.
